Transparency
As the plan sponsor of an employee group health plan, Millennium Contractors, LLC is required to share information regarding the cost of in-network and out-of-network covered medical services under the Transparency in Coverage Rule.
Those details may be accessed UnifiedGrp.com/Resources . At the Unified Group website, click on the Resources tab at the bottom of the page. Once on the Resources Page you will see a tab that says Transparency. From the Transparency tab it will then take you to Machine Readable Files and shows the networks available. You will need to click on our company’s Employer Identification Number (EIN) to search for our specific network arrangements. The EIN you should use is 35-2077779.
